We establish a diagnosis, identify cognitive & functional strengths & weaknesses, provide support and education, cognitive rehabilitation training, identify resources to remain at home, help determine if higher level of care is needed, assess the need for assistance with finances, medical decisions, & driving, and offer recommendations for managing difficult behaviors.
We use objective Cognitive & Decision Making Capacity Assessments to develop a plan for managing challenges and improve quality of life. We partner with individuals, families and care teams to address needs and connect you to resources. We see all patients in their own homes. This allows for a more complete and inclusive evaluation of functional abilities, resulting in highly personalized recommendations that are specific to the patient, family and situation. Performing assessments in the home allows us to formulate tips for how to incorporate recommendations. We also provide telehealth.
